As a Paediatric trainee, I worked at different departments in the paediatric field including PICU, ward , Neonatal ICU , resuscitation of newborns on delivery , our oncall is 28 hrs, we do 10-12 oncalls/month, my duties included the following :
1-Evaluating patients at the emergency department, deciding whether to admit the patient or not
2-Managing emergency situations at EMG department(convulsion, Status Asthamticus , G6PD crises , arrested cases(CPR)
3-Taking histories+performing complete physical examination for admitted patients +writing them on the system
4-Doing the appropriate labs investigations +radiology for patients +putting an Appropriate management plan in discussion with specialist
5- Calculation of different medications doses including infusion pumps at ICU
6-Discussion with the patient and the patient families , explaining the situation and expected stay, management plan , answering any questions they have
7-Observing patients during the call, observing any deterioration and taking the appropriate steps to keep the patients hemodynamically stable, and to find out the reason or any hemodynamic instability , sometimes this involves contacting the specialist oncall
8-during my residency I performed multiple procedures including the following:
IV cannula insertion or paediatric+neonates
ABGS in NICU+paediatrics
Intraosseous access
exchange transfusion in NICU
chest tube insertion in NICU+PICU
LP paediatrics+Neonates
taking urine culture by catheter or suprapubic aspiration
taking blood culture
BMA + giving intrathecal methotrexate(observed multiple times)
9-I was the senior resident so I Used to put the monthly oncalls schedule
10- Presentations , 2-3 presentations per month for each resident, 2 presentations were presented per week
11-presenting any event on patients and how it was dealt with +new admissions at the morning report meeting , discussion of further management plans +what went right or wrong
12-Discussing an appropriate discharge plan with specialists for patients being discharged
13-Skilled at working independently and collaboratively in a team environment
14-Worked well in a team setting, providing support and guidance
15-Managed time efficiently in order to complete all tasks within deadlines
16-Demonstrated respect, friendliness and willingness to help wherever needed.
